The National Disability Insurance Scheme (NDIS) provides funding to eligible individuals with a disability so that they can access the supports and services they need to live and enjoy their lives.
NDIS funding is designed to improve access to support services, foster independence and skill-building, and provide access to jobs and opportunities in the community, with the aim of improving quality of life for individuals with a disability.
If you think you might be eligible for the NDIS, you can download an Access Request Form (ARF) and Supporting Evidence Form from the NDIS website. You can then work with your GP or other healthcare professional to complete the information, gather the supporting documents required and submit the request to the NDIS.

The support that you are able to use NDIS funding for will depend on and be specified in your plan. Supports are divided into three main categories covering core, capital and capacity building needs.
Core supports help participants with their everyday activities and daily living needs.
Capital supports fund higher-cost pieces of assistive technology, equipment, and home or vehicle modifications.
Capacity building supports help participants to build their independence and skills.
